The order form images above are 3 1/8" by 2 1/4". These forms MUST be printed in the original dimensions and cut out. PLEASE DO NOT ENLARGE THEM.
(It also is possible to right click [PC] or click and save as [Mac] the image and print it from any image editing program.)

We are aware that there are many different computers, printers and graphic programs, each with a variety of user settings. However, it is not possible for our staff to process orders on forms that are of different sizes. Oversized order forms will be returned.

A good rule of thumb is:  If the form, when printed out is larger than your check, it is too big and needs to be resized.

Remember:

You must use a separate check and order form for each show that you are ordering.

Please print your Customer ID CLEARLY.

Please put your name and address on the card in case we need to return your order and check to you due to a shortage of tickets.
